All new patients (and established patients who have not
been to our office within the past year)
need to complete the Patient Informaton form. For your convenience, you may complete our
Patient Information Form prior to your visit. The form has been designed so
that you can open it in your web browser, save it to your computer and fill
it out before printing it. You can
also print the form and fill it out by hand.

Once you have filled the Patient Information Form out
completely, you may email it to our office at appts@dermlaser.com, fax it to our
office at 210.614.0530, mail it to the address above or bring it with you to your appointment. Please arrive at least 20 minutes early
for your appointment and be sure to bring
a current insurance card with you.
Our office is open to serve you
Monday through Friday from 8:00 am til 5:30 pm.
New Patients are always welcome.
To schedule an appointment,
please call (210) 614-3355, select option “2” or click on appointments. Our staff is happy
to assist you in any way and every effort will be made to accommodate your
schedule.
If you are unable to keep a scheduled appointment, please
notify the office as early as possible.
This appointment time has been reserved for you and failure to cancel
your appointment within 24 hours (at least 1 business day) will regretfully
result in a missed appointment charge.

to schedule a consultation with one of our board-certified dermatologists of
physician assistants. They will be happy to
discuss any questions or concerns you might have regarding our cosmetic
procedures.
As a
service to patients, our office will bill the patient’s insurance (both
primary and secondary) for services rendered. Co-payments and payments
for treatment not covered by insurance are due at time of service. Charges, co-payments, co-insurance and
deductibles are required at the time of the visit.
We
accept cash, check, Visa/MasterCard,
American Express and Discover.
If your insurance company is an HMO and requires you to be referred to a
specialist by your primary care physician, the referral must be received by
our office prior to your appointment.
Please call our office the day before your scheduled appointment to
verify that we have received your referral.
